By submitting this application and paying the $________ fee I hereby request permission until 31 December, 2004 for access from Big Run Road across property owned by Hutton and Loyd Tree Farm, LLC ("H&L") to Big Run Lake for purposes of fishing by boat or from the bank only on the H&L portion of the lake. Please check one of the following membership types.

	$50 Annually
	Names Grandchildren under 12 years of age and accompanied by me.
	Additional $5/Child, Annually

______	Myself and 1 unnamed guest accompanied by me.
	$60 Annually

______	Myself and 2 unnamed guest accompanied by me
	$75 Annually

Other rates are available upon request, please call for details.

I understand that this permission extends only to the type of membership that I have indicated above. I also understand that this fee is an annual fee and is not refundable if the application is accepted by Oliver H. Loyd, manager of H&L, nor can the fee be prorated for a portion of the year.

I acknowledge that I understand where the H&L property line crosses the lake and agree to not cross this line unless I first obtain separate permission from the adjoining party to do so. I agree to abide by all rules and regulations, including size and quantity of fish caught stated in the Kentucky Fish & Wildlife Guide. If I use a motor boat I agree to use only one with a battery operated trolling motor and will not use a boat with a gasoline motor.

I also agree to not use a trotline, jug line or throw line. I also agree to dispose of my trash by taking it out with me or placing it in a receptacle properly designated for trash. I also agree to report in a timely manner to Mr. Loyd the presence of persons fishing at the lake who do not have Mr. Loyd's authorization for access. I will in such cases attempt to obtain the identity of or a description of such persons as well as a description of their vehicles and its license number. I hereby affirm that I and any members of my family and guests whom I allow to use this permit are aware of the dangers of water sports and have the requisite skill and knowledge to fish, boat and use the lake safely and wisely. I hereby acknowledge that I, for myself, my family members and guests, forever release and will hold H&L and its members and managers harmless from and indemnify H&L and its members and managers against any claims of liability should adverse events occur while on the property and/or using the lake.

Note: Please sign and return one copy of this application, a check made payable to Hutton and Loyd Tree Farm, and mail to Charlene Hall at P. O. Box 24, Plummers Landing, KY 41081. If questions call 606-876-3423. Keep one copy for yourself. When I receive the fee and the application I will prepare a wallet size card for you as evidence of the permission. Whenever you use the lake please have the card on your person in case challenged by the Fish and Game Warden or myself. Please get to know the other members of the club. Thank you, Oliver H.
